Format for Submissions
Submissions are to be made electronically on the form below.
They must consist of a brief (maximum 150 word) summary entered into the on-line submission page. A PDF document of no more than two pages (in 12 point font), outlining the response to the challenge, is to be attached. Proponents should provide details in the two-page submission, under these headings: title of project; names of applicants; lead applicant; the organisations they represent; background to the problem; the resilience or Safety-II innovation; how it contributed or will contribute to improved health care; the potential or actual generalisability elsewhere; and a very brief plan for take-up elsewhere.
The panel’s decision will be final; no correspondence will be entered into; payments will be made to the named lead applicant; and successful applicants must agree to participate in activities to promote their application via media releases and other activities. We will invite the lead applicant of each prize-winning entry to the Resilient Health Care Net meeting in Vancouver, Canada scheduled for August, 2017 to accept their awards. Note that expenses for attendance will be met by the successful prize-winners, and are not funded by the Prize.
